In order to update the Parish Register of our Mission,all families are requested to complete the Membership Registration Form and return it to the Director or Trustees.For those who haven't completed the form in the past,please try to fill up all the information and sign the form.If you don't know the dates of any sacraments received ,you can leave those columns blank.At least your name,current address,and the phone number must be filled up.
